Download and fill out this form to reserve a room and or request audio-visual, catering, child care, worship music, or other resources for your event.  If this is a ministry event, you must coordinate with your ministry and cluster leaders prior to submitting the request.  You are still responsible for following up with the church office to ensure that your event needs are covered.

You can print this out, sign it, and send it to the church office to begin the membership process.  Membership also requires attending a Membership Class and filling out a Getting to Know You Form which you can pick up at the Welcome Center table.
